In consideration of being allowed to participate in any way in Special Olympics sports training, competition or fundraising activities, the undersigned acknowledges, appreciates, and agrees that:
1. Participation includes possible exposure to and illness from infectious and/or communicable diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Special Olympics, Inc, Special Olympics Rhode Island, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THENEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
As a volunteer member of Special Olympics Rhode Island, I agree to:
I will follow the policies, rules, and procedures of Special Olympics Rhode Island
I will place the safety and well-being of the Special Olympics athletes as foremost.
I will interact with the Special Olympics athletes in an appropriate and dignified manner that any athlete or individual deserves.
I will represent Special Olympics Rhode Island in a professional manner that presents a positive image to the community.
I will accept challenges using a constructive approach.
I grant Special Olympics permission to use my likeness, voice, and words in television, radio, film, or in any form to promote activities of Special Olympics.
Technology Acceptable Use Policy
INTRODUCTION
Special Olympics Rhode Island (SORI) offers authorized users access to organizational devices, the network, the Internet, and an array of technology resources to promote operational excellence. Authorized users include staff and approved volunteers and will be hereafter referred to as users. Each user is responsible for their use of technology whether it is provided by the organization, or it is a personal device. While using SORI or personal technology resources on organizational property or during business hours (vehicles, fund raising events, sporting events, etc.) as well as using SORI technology resources via remote access and/or at home, each user shall act in a manner consistent with the organization and legal guidelines.
USING SORI TECHNOLOGY RESOURCES
All access to the SORI network shall be preapproved by SORI. The network and Internet access available through SORI serves the purpose of supporting business practices and is for the mutual benefit of all users. The use of the SORI network and Internet access is a privilege, not a right. Users shall not attempt to disrupt the operation of the network or equipment and/or interfere with the work of SORI users. SORI may restrict or terminate any user’s access, without prior notice, upon approval of the Chief Executive Officer or designee. Additional disciplinary action may be imposed.
RESPECT
All users shall respect other SORI users and property when using technology resources by:
Using assigned workstations or devices
Always logging out of accounts at workstations after finishing work
Not disrupting system performance or interfering with the work of other users
Leaving common equipment and surroundings in designated locations in good condition for the next user(s)
Immediately reporting technology issues to the appropriate technical support personnel.
It is the responsibility of the user to:
Use only his or her account(s) and password(s)
Not read, modify, remove, or delete files owned by other users
Use SORI technology resources in a manner that is consistent with the mission of the organization
Use of SORI technology resources for private gain is prohibited.
Support the integrity of technology resources.
Deliberate tampering or experimentation is not allowed, including illicit access
Refrain from using offensive, obscene, or harassing language in communications
Appropriately use licensed software
INTERNET SAFETY AND SECURITY
Information may not be posted if: it violates the privacy of others, jeopardizes the health and safety of others, is obscene or libelous, causes disruption of the SORI operational environment and/or activities.
Users are not to reveal personal information (for example: last name, home address, phone number) in correspondence with unknown parties.
Users are responsible for reporting any inappropriate material they receive.
Users are prohibited from accessing Internet sites that are inconsistent with the organizational mission of SORI.
Users are prohibited from viewing, sending, and accessing illegal material.
Users are prohibited from downloading illegal material or inappropriate content while using organization-owned devices.
Any device placed on the SORI network is subject to discovery under the Freedom of Information Act (FOIA), and possible confiscation by authorities.
SORI reserves the right to monitor and investigate all activities on SORI-owned computing equipment and accounts on the SORI network.
